From the phrasal idea of “work” + “up” meaning to develop or build something up; as a noun, it became common in the early–mid 20th century, especially in American English and medical usage for a set of diagnostic tests.

A work-up is a thorough preparation or a systematic series of examinations/tests to understand, develop, or diagnose something (often a medical evaluation).
